Some VERY important rules:

1) This form must be printed on legal sized paper and be double-sided.

2) Each line must be filled out in its entirety -- no blanks or ditto marks

3) Each petition is for only one county.  If you are collecting signatures from people from multiple counties, you need multiple sheets -- one for each county.
